Child Emergency Plan

A Child Emergency Plan is a writing that gives clear instructions for the care and well-being of your child in the event that something should happen to you.


Power of attorney for property

A Durable Power of Attorney for Property appoints an agent to act for you with respect to your property. The agent has the power to make decisions for you regarding assets and liabilities in your name (for example, filing tax returns). If you are unable to manage your financial affairs, the person you name under your Power of Attorney for Property may act for you.

A Durable Power of Attorney for Health Care appoints an agent to make health care decisions with respect to your health. The agent will direct all types of medical care, and inspect medical records. The agent is given authority to access your health information in a timely manner even in the face of “HIPAA”, the Health Insurance Portability and Accountability Act. If you are unable to manage your health care affairs, the person you name under your Power of Attorney for Health Care may act for you.
